Members start with self-service screening guided by Mindy, then use a secure portal for scheduling, two-way messaging, and pre and post visit education. Care planning becomes a living, shared care plan - reminders, check-ins, intervention tracking and Cortex for Caregivers offering on-demand, llm driven guidance under clinician oversight. Visit efficiency boosting preparation, coordination, and structured data flow to the EHR, freeing teams for high-value care - making this patient & family layer a core pillar of our technology investments: human-centered, measurable, and scalable.

Great outcomes don’t come from single features; they come from a coordinated journey. Mindspan’s Orchestration Engine turns assessments, messages, orders, and follow-ups into a reliable flows. It watches for signals (a new MoCA score, missed appointment, caregiver question), applies clinical and operational rules, and triggers the next best step - without creating extra clicks.
